Cancellation & No Show Policy
Illuminating Life Behavioral Health Cancellation and No Show Policy
This policy is in place to honor our providers' time. You are responsible for attending each appointment and agree to adhere to the following policy:
A 48-hour notice of cancellation is required to avoid a fee being charged. Appointments can be canceled or rescheduled by logging into your client portal or clicking "cancel" within your appointment confirmation email. Insurance companies do not reimburse for missed, late cancel, or no show appointments, these are your responsibility. A late cancellation (less than 48-hour notice) will be charged to your card on file in the amount of $75 for a follow up appointment and $150 for an intake appointment. A no show appointment (10+ minutes late will be treated as a no show) will be charged to your card on file in the amount of $150 for a follow up appointment and $300 for an intake appointment. If you card on file does not process (i.e. insufficient funds, do not honor, closed account, expired card, etc.) any future appointments will be canceled or declined until your balance is paid in full. If this becomes a frequent occurrence, it will result in a no show fee and you will be discharged from the clinic. Refills and medication changes will NOT be made or approved outside of appointments.
All clients are subject to this No Show/Late Cancel policy with the exception of those utilizing Medical Assistance (MA) or a Prepaid Medical Assistance Plan (PMAP). MA and PMAP do not allow providers/clinicians to charge No Show/Late Cancel fees, and due to this exception, we have the following policy regarding clients with MA/PMAP:
Three no show or late cancellations (less than 48-hour notice) are grounds for termination of treatment and/or therapy.
This policy is in place to ensure each MA and/or PMAP client is able to hold themselves accountable, for our clinician's time to be respected, and to maintain each client's access to receive care.
**If an appointment is canceled or no showed for any reason, you are responsible to reschedule as soon as possible as refills will not be approved until you are seen at your next appointment.
