Cancellation & No Show Policy
Illuminating Life Behavioral Health Cancellation and No Show Policy
This policy is in place to honor our providers' time. You are responsible for attending each appointment and agree to adhere to the following policy:
A 48-hour notice of cancellation is required to avoid a fee being charged. Appointments can be canceled or rescheduled by logging into your client portal or clicking "cancel" within your appointment confirmation email. Insurance companies do not reimburse for missed, late cancel, or no show appointments, these are your responsibility.
Fees:
Late cancellation (less than 48 hours):
$75 for follow‑up appointments
$150 for intake appointments
No‑show (10+ minutes late):
$150 for follow‑up appointments
$300 for intake appointments
Fees are charged to the card on file. If your card declines, future appointments will be canceled until the balance is paid. Repeated declined payments may result in discharge from the clinic. Medication refills or changes are not provided outside of appointments. Refills and medication changes will NOT be made or approved outside of appointments.
MA/PMAP Clients
Clients with Medical Assistance (MA) or Prepaid Medical Assistance Plans (PMAP) cannot be charged no‑show or late‑cancel fees.
Instead:
Three late cancellations or no‑shows (less than 48‑hour notice) may result in termination of services.
This ensures accountability, respects clinician time, and helps maintain access to care.
This policy is in place to ensure each MA and/or PMAP client is able to hold themselves accountable, for our clinician's time to be respected, and to maintain each client's access to receive care.
**If an appointment is canceled or no showed for any reason, you are responsible to reschedule as soon as possible as refills will not be approved until you are seen at your next appointment.
